Abstract
ObjectiveTo consider whether institutional abuse could recur and how that risk may be reduced.ConclusionsVulnerable populations are likely to remain at risk unless failures in care, and the subsequent abuse, are recognised. Some suggestions are made including an open organisation where the status quo is contested and innovation welcomed. To speak up individually is frightening, unless supported by a shared culture that agrees to do so.
